REQUIRED DOCUMENTS
1. Birth Certificate
2. Current Physical from medical provider (within 6 months of season begining)
3. Proof of Residency/Utility bill (City bill, Power bill, Water bill, Internet bill.
*Must show address of service.
4. Document that proves the grade of the player you are registering.
**The purchase of a new jersey is optional for returning players but required for all others.
Jersey cost is $50

Saturdays: July 22nd and 29th 9am - noon
Weekdays: July 17th, 19th, 25th, 27th 6pm - 8pm
Pre Season Skills Camp - Fundraiser
June 20, 21, 22 7-8pm
Pioneer East/West Park in Brigham City
Sign up for the camp with your season registration or
come to the park and sign up the day of.
First day of practice July 31st
Preseason Jamboree August 12th
First season game August 19th
**TRYOUTS**
Some age groups we will be required to have a Competitive team.
If this is the case, you will be contacted regarding tryouts.
EVERYONE WHO REGESTERS TO PLAY
WILL BE PLACED ON A TEAM.
